Job Description

• Completing needs and goals of assessments of young people referred into the intensive home treatment team. Conducting home visits and other community contacts to young people presenting in crisis or have been discharged from tier 4 inpatient services and the Health Based Place of Safety. We are an agile team and work across Kent and Medway. We continually assess risk and conduct mental state examinations of the young people on our caseload. Safeguarding our young people and families is paramount and knowledge of the referral process is essential. We try and prevent admission to tier 4 services, by offering an intense therapetuic package of care to the child and family, we also offer support for young people who require more intensive support in the community and to work alongside our locality teams and other professionals. We work closely with locality CAMHs teams, social care, police and education by attending joint reviews, medical reviews and network meetings.

Requirements

• Nurse – MH, Paed, LD, Social worker, OT if relevant mental health (ideally ChYPMHS) experience. Therapeutic skills / interventions offered – Mental State examinations, risk assessments, care plans, safety planning with young people and care givers. We provide DBT informed interventions, Behavioural Activation, Motivational Interviewing and other relevent brief interventions.
