Job Description
- A Qualified practitioner with ability to manage caseload within community setting with support and supervision. Assessment of new referrals to service. Liaising with clients, carers and families via telephone, video conferencing or face-to-face. Covering Duty. Taking part in regular MDTs
Requirements
- Experience of working within a community older adult or Memory Service team. Qualified mental health practitioner who has knowledge of working with and managing a caseload autonomously in the community.
